Symptoms of Early Dementia Questionnaire

Cognitive impairment is a condition caused by changes in the brain’s nerve cells, leading to a decline in brain function. It affects the patient’s memory, understanding, language, learning, calculation and judgement abilities, among others. Early detection of cognitive difficulties in older adults can help manage the condition and slow its progression.

The Symptoms of Early Dementia-11 Questionnaire was developed by the Haruhiko Yamaguchi Research Laboratory and contains 10 questions that help identify symptoms of cognitive impairment early. Please complete this questionnaire based on the older adult’s experiences over the past month.
